Community Resource Navigator

The TriCounty Active Adult Center (TRAAC) is hiring a full-time Community Resource Navigator. This position assists and enhances the well-being of older adults by supporting and connecting them with invaluable internal, local, county, state, and federal services, resources, and benefits that enable their continued independence and self-sufficiency. Additional responsibilities include volunteer management, communications/social media, and community relations/engagement.


  • Preferably a Bachelor’s Degree, with a Social Work major or AA Degree in Human Services, or 6 years relevant experience.
  • Extensive knowledge of programs, benefits and resources supporting eldering adults.
  • Friendly, outgoing and relational.
  • Successful background in working with older persons.
  • Organizational skills necessary to perform the Community Resource Navigator position.

  • WordPress (preferred)
  • Comfortable learning new programs and technology


  • Become familiar with local, county, state and federal assistance programs in order to assist older adults in obtaining assistance, fill out required forms, etc. These programs include, but are not limited to:
    • Local food pantries
    • Housing assistance programs
    • LIHEAP
    • SNAP
    • SNFMP (Farmers Market Vouchers)
    • Veterans’ Affairs
    • Transportation–TransNet
  • NAT-E forms
    • Coordinate distribution of nutrition surveys to participants
    • Enter data into the online database
  • Income tax appointment scheduling and coordination
  • PA-MEDI appointment scheduling
  • Senior Food Box delivery and distribution
  • Volunteer Management
    • Recruitment & orientation
    • Coordination
    • Recognition
  • Communications and community relations
    • Attend community events to represent and promote the TRAAC
    • Cultivate relationships with referral agencies and organizational contacts
    • Maintain website
    • Coordinate quarterly newsletter production
    • Post on social media
  • Maintain on-going contact with members experiencing difficulties (hospitalization, nursing home convalescence, death of a spouse, etc.) affecting attendance at the center. These may include hospital, nursing home or residence visitations.
  • Identify elderly members of the community who could benefit from the Center’s services.
  • Plan with the Assistant Executive Director appropriate activities and programs for elderly members.
  • Assist the Executive Director in the implementation of procedures for handling disruptive client behavior.
  • Maintain case notes, resource file/s and all necessary records and documentation related to assistance programs offered to elderly clients and members.
  • Complete required statistical reports for submission to funding sources.


Job Type: Full-time


  • $35,000 – $40,000 per year

Schedule:                                Monday to Friday, 8am – 4pm

Source of Supervision:           The Assistant Executive Director for working
directions and assignments.
